A holistic assessment, utilising a biopsychosocial model, will determine service user needs. This approach quantifies clinical, functional, and risk management requirements, ensuring a comprehensive and person-centred understanding of their support needs.
In parallel, we will use validated scales such as the SOFA¹, to obtain a baseline and dynamic review of our clients’ improved autonomy.
